The following is a summary of some of the information you will need.
Identification (name, date of birth, Social Security Number) for yourself, and your current and any former spouse, or your minor children.
Your banking information, to allow for direct deposit of benefits.
Names, addresses, phone numbers and dates of treatment for all doctors, hospitals, or other places you have received treatment.
